During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 03/21/2025 and completed on 04/09/2025 in response to a complaint received by the licensing office on 03/10/25 relating to allegations of ratio and staffing. The Department requested that the provider complete an IPOC as result of the last inspection related to ratio and background checks. The inspector reviewed 2 children?s records and 3 staff records electronically on 04/09/25. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/16/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-350-B-2
The required ratio for children ages 16 to 24 months is one staff per five children.

1. Review of documentation revealed that staff #1 and staff #2 were alone in classroom #1 with 16 children (youngest child 17 months) from approximately 12:30 pm - 3:30 pm.
2. Staff #1 and staff #2 were observed supervising 12 children on the playground (youngest child 18 months) at approximately 4:25 pm.
Plan of Correction: Classrooms will be staffed to meet state ratio requirements. In the event ratio is unable to be met, children will not be accepted into care until another staff member is available to ensure compliance.

The required ratio for children ages 3 years up to school age is one staff per 10 children.

1. Staff #3 was observed supervising 14 children (youngest child 3 years old) on the playground from approximately 4:25 pm to 4:40 pm while staff #4 left the playground to conduct a tour.
2. Review of documentation revealed that from approximately 3:12 pm to 3:39 pm staff #3 and staff #4 were supervising 23 children (youngest child 3 years old).
Plan of Correction: Classrooms will be staffed to meet state ratio requirements. In the event ratio is unable to be met, children will not be accepted into care until another staff member is available to ensure compliance.
