Inspector Notes

A code compliance inspection was conducted on Tuesday, May 16, 2023. The inspector was on site from4:20pm to approximately 5:00pm. There were 24 children in attendance with three staff directly supervising. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. The children were observed waking up from nap, taking bathroom breaks, washing hands, and later during a group activity. The center is equipped with toys and supplies and items are available to the children. The center provides transportation and all requirements were found to be in compliance.All areas of the facility including classrooms, hallways, the outdoor play area, and bathrooms were inspected. Five children's records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interview, the center did not ensure one staff
had a required background check within the required timeframe.

Evidence: 1) The record of Staff #3, employed on 11/09/20, did not contain the results of the
central registry check.

2) During interview, a member of management stated the central registry check for Staff #3
could not be located.

The search of the central registry check is required within 30 days of employment or commencement of service.
Plan of Correction: A new request will be submitted to the Office of Background Investigation. In the future, staff will obtain a central registry check within 30 days of employment.
