Inspector Notes

A monitoring inspection was initiated on 12/4/23 and concluded on 12/4/23. There were nine children present with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed. The inspection started at 10:20 am and concluded at 11:15 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review the center failed to ensure that staff who have lived out of the state of Virginia in the past five years have a criminal record check and sex offender check before first day of employment and a CPS check requested within 30 days of employment.
Evidence:
Staff #1 listed a residence outside of Virginia in the past five years and there are no record checks for that state on file.
Plan of Correction: The director will ensure that all staff that have lived outside of the state of Virginia in the past five years have background checks from that staff.
