Inspector Notes

An unannounced monitoring inspection was conducted today from 11:15am-12:55pm. There were 44 children (infants-5yrs.) directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, playing on the playground, and eating lunch. There was an adequate supply of materials for the children. The center was clean and organized.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on record review, the center did not obtain a repeat fingerprint based background check for a staff member every five years as required.

Evidence:
Staff#1's record contains a fingerprint check result dated 2/26/2018, the background check expired on 2/26/2023.
Plan of Correction: We got the appointment until last week and she's going today. Fingerprint attached.
Standard 8VAC20-780-210-A
Based on observation, the center did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence:
There was no lead qualified teacher in the Preschool classroom. Staff#5 is not lead qualified.
Plan of Correction: That lead teacher is qualified, but her papers were not presented properly. She has been working with kids and having trainings since 1999.
Standard 8VAC20-780-500-B
Based on observation, the diapering mat was not nonabsorbent.

Evidence:
In the Two's classroom, the diapering mat was no longer nonabsorbent due to a tear of approximately 1 inch in diameter in the material.
Plan of Correction: MAT was changed for a new one on 5/2/2023.
