Inspector Notes

An unannounced monitoring inspection was conducted today from 12:35pm-2:00pm. There were 32 children (infants-5yrs.) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping. There was an adequate supply of materials for the children. The center was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on record review, the center did not ensure that all sworn disclosure statements were updated every 5 years.

Evidence:
Staff #3's record contained a Sworn Disclosure Statement that was last updated on 5/29/2018.
Plan of Correction: Already signed and posted in her file.
Standard 8VAC20-780-160-C
Based on record review, the center did not obtain a Tuberculosis screening or test result update every 2 years as required.

Evidence:
Staff #4's record contained a TB screening form that was last updated on 6/10/2021.
Plan of Correction: 11/20/2023 Teacher took TB test and is waiting for results. The TB test expired on 6/10/2023.
Standard 8VAC20-780-245-J
Based on record review and interview, children that have emergency medication on-site were not always in the care of a staff member with MAT certification.

Evidence:
Child#1 has an emergency medication on site, the child was not in the care of a staff member currently certified to administer medication.
Plan of Correction: Two teachers are now MAT certified as of 11/18/2023.
Standard 8VAC20-780-260-A
Based on a review of documentation and interview, an annual inspection report from the appropriate fire official was not completed.

Evidence:
The fire inspection documentation available for review was dated 6/14/2022.
Plan of Correction: Fire department contacted on 11/14/2023 and we are waiting for them to give us an appointment.

Based on review of four staff records, the facility did not ensure each staff record contained written information to demonstrate that the individual possesses the education, staff development, or certification required by the job position.

Evidence:
The records for Staff 3, Staff 4, and Staff 5 did not contain documentation that the individuals possess the education, or staff development training, or certification required by their job positions.
Plan of Correction: Files corrected adding diplomas and/or copies of trainings.
