Inspector Notes
An unannounced monitoring inspection was conducted 4/18/2024 from approximately 9:50am-12:00pm. There were 5 classrooms with total of 34 children and 5 teachers, all ratios were in compliance. Children were observe listening to music, eating lunch and outside on the playground. Positive interactions with staff and handwashing was observed. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, postings and policies were inspected. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

7Evidence: No date on current emergency plan.
Evidence: No staff had MAT certificate available at inspection.
Evidence an outlet in Infant room and in lunch table area had unused/exposed outlets.
Evidence: In 4:5 diapering stations, diaper changing pads had tears or holes, rendering them absorbent.
Evidence: Child #1 has prescription medication and the center does not have parental authorization to administer, on file.
Evidence Child #1, Child #2, Child #3 did not have written parent authorization noting any known adverse reactions in their files.
