During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was conducted on 9/8/2025 from 10:35am to 11:55am. There were 24 chldren and 5 staff present. The inspector reviewed 4 child records and 4 staff records on-site.The inspection included document review, interviews, observations, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 9/16/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

At the time of the inspection, Staff #1 was six months overdue for a subsequent TB screening.
Plan of Correction: All staff records have been reviewed to insure they have their updated TB test. The particular teacher who did not have it at the time of the inspection was scheduled to have their last day with us on September 8, 2025. The rest of the staff has the updated test or has since made arrangements to complete it.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all childrens allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room where children are present.

Child #1, enrolled for more than 6 months, had documented allergies that were not indicated on the written list accessible to staff. The written list that was posted was not dated.
Plan of Correction: The childs records have been reviewed and all allergy lists have been updated to include all allergies. The lists were also dated.

REPEAT VIOLATION
Prepared infant formula shall be dated.

In Classroom #1, 1 out of 3 bottles were not dated.
Plan of Correction: We have insured that every morning the teacher has a reminder to check all bottles/food containers for a name and date.
