Inspector Notes
A subsidy inspection was initiated on 04/19/2023 and concluded on 04/19/2023. The inspector was on site at the center from approximately 11:15am to 2:15pm. There were 26 children in care with 6 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

1. The record of staff #1 (employment date: 9/22/2022) had documentation of central registry results dated 11/12/2022.
2. The record of staff #4 (employment date: 3/23/2022) had documentation of central registry results dated 11/12/2022.
3. The record of staff #5 (employment date: 1/2/2023) had documentation of central registry results dated 2/4/2024.
Evidence:
1. The record of staff #2 (employment date: 9/19/2022) did not have documentation of out of state criminal history record results, and sex offender registry check results.
2. The record of staff #2 indicated that the staff member has lived outside of Virginia within the last 5 years. Staff #2 confirmed that she has lived outside of Virginia within the last five years.
Evidence:
1. The record of staff #1 (employment date: 9/22/2022) had documentation of a completed sworn statement or affirmation dated 10/6/2022.
2. The record of staff #4 (employment date: 3/23/2022) had documentation of a completed sworn statement or affirmation dated 4/7/2022.
3. The record of staff #5 (employment date: 1/2/2023) had documentation of a completed sworn statement or affirmation dated 1/24/2023.
Evidence:
1. The record of child #1 (enrollment date: 10/14/2022) did not have documentation of immunizations.
2. The record of child #2 (enrollment date: 3/17/2023) did not have documentation of immunizations.
3. The record of child #3 (enrollment date: 1/17/2023) had documentation of immunizations dated 2/27/2023.
Evidence:
There were several containers of cleaners located in the center?s closet and kitchen. Both the closet and the kitchen were unlocked.

Evidence:
The inspector observed an infant sleeping in a crib with a blanket underneath the infant?s head as if it were a pillow. The same infant also had a blanket on top of the infant and another blanket and cloth bib on the side of the infant.
Evidence:
The infant classroom used a trashcan located outside of the infant rooms back door. The trash can was not foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
The licensing inspector observed two children sitting in a feeding table without the protective belt fastened. There were no protective belts installed in the feeding table seats as required by the manufacturer.
