Inspector Notes

An unannounced renewal inspection was conducted on February 6, 2023 from 10:00 am until 11:30 am. There were 5 children present (12 points) 2 caregivers. The home, children's records, caregiver records, household member records, bathroom, napping area, evacuation logs, the first aid kit and menu's were reviewed. Standard 8VAC20-800-340-C
Based on observation, the provider failed to ensure that the bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap and paper towels/

Evidence:
The bathroom was not supplied with paper towels.
Plan of Correction: Paper towels were placed in the bathroom during the inspection.
Standard 8VAC20-800-50-A
Based on observation, the provider failed to keep a written record of children in attendance each day.

Evidence:
The attendance record had not been filled in for Monday, February 6, 2023.
Plan of Correction: I will take and maintain attendance daily.
Standard 8VAC20-800-60-B
Based on a review of children's records, the provider failed to obtain all required documentation of each student enrolled.

Evidence:
Child #1 and #2's records did not contain documentation of the review of the child's emergency contact information as required by 8VAC20-800-780-B.
Child #5's record did not contain documentation of a the name, address and telephone number of each parent who has custody.
Plan of Correction: I will get the information from parents.
