Inspector Notes

A monitoring inspection was conducted on 9/5/2023. There were a total of 5 children ( 11 points) observed with 2 caregivers present. The children were engaged in the following: toy play, puzzles, shapes, words, bathroom breaks, hand washing. The home, 5 child records, 4 caregiver/household member records, evacuation drills, attendance, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-120-B
Based on a revie of caregiver records, 1 caregiver's record does not contain all the required information.
Evidence: Caregiver #2, date of employment unknown does not have documentation of date of employment, name. address and telephone number of a person to contact in an emergency and documentation of 2 references.
Plan of Correction: The caregiver will complete the required documents, the provider will obtain 2 references for the caregiver.
Standard 8VAC20-800-200-C
Based on a review of caregiver records, 1 caregivers record does not have documentation of the orientation
Evidence: Caregiver #2, date of first employment unknown, does not have documentation of orientation .
Plan of Correction: The provider will provide the orientation and file the document in the caretaker's record.
Standard 8VAC20-800-240-A
Based on observation of the outside playground equipment, the equipment is not maintained in a safe and operable condition.
Evidence: The far left swing set has a swing where the chain is rusted and plastic covering on the chain is cracked and peeling.
Plan of Correction: The swing will be removed or the chain will be replaced.

Based on a review of children records, 1 child's record did not contain all the required information.
Evidence: Child #1 record, date of enrollment 5/16/2023 does not have documentation of insurance policy number and written authorization for emergency medical care.
Plan of Correction: The Provider will obtain the information from the parent and write on the child record form.
