Inspector Notes
A monitoring inspection was conducted on 9/5/2023. There were a total of 5 children ( 11 points) observed with 2 caregivers present. The children were engaged in the following: toy play, puzzles, shapes, words, bathroom breaks, hand washing. The home, 5 child records, 4 caregiver/household member records, evacuation drills, attendance, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence: Caregiver #2, date of first employment unknown, does not have documentation of orientation .
Evidence: The far left swing set has a swing where the chain is rusted and plastic covering on the chain is cracked and peeling.
Evidence: Child #1 record, date of enrollment 5/16/2023 does not have documentation of insurance policy number and written authorization for emergency medical care.
