Inspector Notes

An unannounced monitoring inspection was conducted on February 21, 2024, from 10:00am-11:15am. At the time of entrance, ( 7 ) children ( 15 ) points were in care with ( 1 ) caregiver present. The provider arrived about 15 minutes into the inspection. The inspector reviewed 4 children?s records and 2 caregiver/household member records. Children were observed engaged in individual play .and group story and activity. No medications were on site.
Discussed the use of a substitute provider and the requirements. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Based on observation and provider interview, the current inspection report dated 9/5/2023 is not posted in a conspicuous place.
Evidence: The last inspection report dated 9/5/2023 was not posted for the parents to view.
Plan of Correction: The provider will post the inspection report in a conspicuous place for the parents.
Standard 8VAC20-800-180-A
Based on a review of caregiver records, the provider has not obtained every 2 years from the last screening a Tuberculosis screening.
Evidence: Caregiver #1 has not updated their TB screening. Their current Tb screening is dated 12/27/2021
Plan of Correction: Caregiver will get the TB screening.
Standard 8VAC20-800-240-A
REPEAT VIOLATION
Based on observation of the outside playground equipment, the equipment is not maintained in a safe and operable condition. Evidence: The far left swing set has a swing where the chain is rusted and plastic covering on the chain is cracked and peeling.
Plan of Correction: A new swing has been purchased, a workman will install the swing in a few weeks. The rusted swing has been removed.

REPEAT VIOLATION
Based on a review of children records, 2 child's record did not contain all the required information.
Evidence: Child #1 record, date of enrollment 5/16/2023 does not have documentation of insurance policy number and written authorization for emergency medical care. (Repeat)
Child #2 record, date of enrollment 9/18/2023 does not have documentation of insurance policy number and written notice of liability insurance disclosure.
Plan of Correction: Parents will complete the information.
