Inspector Notes
An unannounced monitoring inspection was initiated on 7/28/25 and completed on 7/28/25 in response to a self-report received by the licensing office on 7/10/25 relating to supervision. This inspection included document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff #1-#3 did not ensure the care, protection, and guidance of child #1 (14month old) when the child was left outside on the toddler playground alone for approximately 11 minutes on 7/9/25 at approximately 4:37pm.
Staff #1-#3 did not ensure the care, protection, and guidance of child #1 (14month old) when the child was left outside on the toddler playground alone for approximately 11 minutes on 7/9/25 at approximately 4:37pm.
Plan of Correction: Center administration followed all incident policies and protocols regarding this incident. Administration followed internal staff incident policies regarding this as well. Training on supervision was done again and further procedures were implemented to ensure future compliance.
