Inspector Notes

An unannounced monitoring inspection was conducted on 10/12/2022 from 1:45pm to 3pm. There were 30 children (5 months to 4 years) directly supervised by 5 staff. The physical plant, 3 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed taking naps. Diapering procedures were observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on observation, the facility did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence: observed that there was less than 6 inches of resilient material around the climbing equipment on the preschool playground. There was approximately 2-4 inches in several places.
