Inspector Notes

An unannounced monitoring inspection was conducted on 5/31/2023 from 12:40 pm to 2:30 pm, on-site and at the main office to view staff records. There were 23 children in two groups under the care of three staff. The Licensing Inspector reviewed four children?s records, one medication, three staff records, and other program records. The children were napping during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center failed to be maintained at no longer than 68 degrees Fahrenheit (F).

EVIDENCE:
The air temperature measured 63.1 degrees F on a digital thermometer.
Plan of Correction: The staff adjusted the thermostat during the inspection.

Based on observation, the center failed to have some cleaning chemicals kept in a locked area.

EVIDENCE:
Bottles of bleach were stored in the unlocked staff bathroom in an unlocked cabinet.
Plan of Correction: The center will obtain a child-proof lock for the cabinet.

REPEAT VIOLATION

Based on observation, the center failed to ensure that a child record had some required information.

EVIDENCE:
The record for Child 1 did not have a parent phone number.
Plan of Correction: The center will get the information and update the child?s record. New child records will be reviewed for completeness from now on.
