Inspector Notes

An unannounced inspection was initiated on 08/05/2025 and completed on 08/05/2025 in response to a complaint received by the licensing office on 07/23/2025 relating to allegations of staff qualifications and training and programs. The inspector reviewed 1 child?s record and 5 staff records on-site. The inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Staff 3 completed 10 of the required 16 hours of annual training from 04/2024 to 04/2025.
Plan of Correction: They will have all training hours by 8/15/25.
