Each staff member shall submit documentation of a negative tuberculosis (TB) screening that has been completed within 30 days prior to employment.

Staff #1 (date of hire: 1/21/2026) has submitted a TB screening that is dated 8/11/2025.
Plan of Correction: We will have them complete a new screening.
Standard 8VAC20-780-160-C
The center shall obtain a subsequent tuberculosis (TB) screening from each employee every two years from the date of the initial screening.

Staff #2 most recent TB screening is dated 2/8/2024. The screening has been expired for approximately 1 month.
Plan of Correction: They will go to complete a new screening.

REPEAT VIOLATION
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

An unlocked cabinet with various cleaning materials was observed in the bathroom of the red classroom.
Plan of Correction: Staff will be reminded that the cabinet needs to be locked even when they are using the cleaning materials.
