A code compliance inspection was initiated on 5/3/2023 and concluded on 5/5/2023. There were 100 children present, ranging in ages from 2 to 4, with 18 staff supervising and 1 administrator. The inspector reviewed compliance in background checks and requirements for child day centers operated by religious institution exempt from licensure. A total of 8 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on inspection and interview with staff, the center has not established and implemented procedures for a daily simple health screening and exclusion of sick children by a trained person.

Based on staff interview the center had not established procedures ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. Reports of deaths shall be submitted no later than one business day after the death occurred.

Based on record review and staff interview the center did not ensure to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff #1 and Staff #7 indicated on the sworn statement as to having lived outside of the state of Virginia within the last five years. The staff person in charge confirmed that the center had not obtained all required out of state background checks.
