Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated and concluded on 3/30/2022. There were 81 children present with 16 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. Staff #1 (Date of hire 9/23/2022) did not have current certification in cardiopulmonary resuscitation (CPR).
2. Staff #4 (Date of hire 2/28/2014) did not have current certification in cardiopulmonary resuscitation (CPR).
Evidence:
1. Staff #1 (Date of hire 9/23/2022) did not have current certification in first aid appropriate to the ages of children in care.
2. Staff #4 (Date of hire 2/28/2014) did not have current certification in first aid appropriate to the ages of children in care.
Evidence:
1. Staff A completed the health and safety update course 3/19/2021, but did not complete it for 2022.
2. Staff B completed the health and safety update course 10/02/2019, but did not complete it for 2022.
