Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 3/30/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 81 children present, ranging in ages from infants to 5 years old, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Based on review of five staff records, the center did not obtain the fingerprint results prior to date of hire.
Evidence:
1. Staff #3 (Date of hire 3/25/2021) had fingerprint results dated 3/31/2021.
Plan of Correction: Will review all dates on paperwork to make sure dates are correct.
Standard 8VAC20-770-60-B
Based on record review, the center did not have a completed sworn statement or affirmation prior to date of hire.
Evidence:
1. Staff #3 (Date of hire 3/25/2021) had a sworn statement signed on 3/30/2021.
Plan of Correction: Will review all dates on paperwork to make sure they are matching.

Based on review of five staff records, the center did not obtain the central registry report within 30 days of employment.
Evidence:
1. Staff #2 (Date of hire 1/24/2022) had a central registry report dated 3/23/2022.

Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of attendance.
