Inspector Notes

A Subsidy Health and Safety inspection was conducted on 7/13/2023 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Evidence:
1. Staff #4 took the health and safety update course on 6/06/2022, but did not take it in 2023.
Plan of Correction: Staff obtained the updated certificates of training.
