Inspector Notes

The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 12/08/2023 relating an injury that occurred at the center not being reported properly. Interviews were conducted with staff and the 12/04/2023 accident report was reviewed.
The information gathered during this investigation determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on interviews, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurred while the child is under the supervision of the center and required outside medical attention.
Evidence:
1. On 12/4/2023, Child #1 fell off the monkey bars at the center, resulting in an injury that required outside medical attention.
2. On 12/7/2023, Child #1's parent reported the injury to center staff in a note and verbally. Staff #1 confirmed that she spoke with the parent on 12/07/2023.
3. On 12/12/2023, the center informed the department's representative of the 12/4/2023 injury.
Plan of Correction: Staff were formally notified to always notify management with in 10 minutes of an accident in the classroom even if not in building.
