Inspector Notes

A monitoring inspection was conducted on 03/28/2023 from 2:40 pm until 3:50 pm. There were four children present, ranging in ages from eight months to eight years old, with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on the review of five child records, the program failed to ensure a child's record contain all required information for one child.

Evidence:
The record for Child #3 was missing the following information: sex, address, parent's name and address and phone number, doctor's name and phone number, two emergency contacts, and authorization to pick up.
