Inspector Notes

A monitoring inspection was initiated on 11/30/2023 and concluded on 12/01/2023. There were 3 children present, ranging in ages from 3 months to 3 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a review of children's records, the provider failed to ensure that parents were provided the required information for parents before the child's first day of attendance.

Evidence:
1. The record of Child 1 (DOE 05/03/2022) did not contain the parent's written acknowledgement of receipt of information regarding the liability insurance declaration.
Plan of Correction: I will make sure the paperwork gets to the parents today and all the papers are in the kid's files.
