Inspector Notes
An unannounced renewal inspection was conducted along with Licensing Inspector Christina Jones on 6/5/24 from 12:50 p.m. to 1:50 p.m. with the provider. There were six children in care, ranging in age from six months to five-years-old, for a total of 14 points. Children were observed napping. Three child records, two staff records, and one household member record were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Violations
5Evidence: The TB screening form for Staff # 1 had a tuberculosis screening on file dated 3/24/2022.
Evidence: Staff #2 had documentation of 3 hours of annual training, Staff #2 confirmed there were no additional training hours completed for the year.
Evidence: The outdoor plays space had small plastic pieces that were broken from toys throughout the playground on the ground, a piece of wood with four protruding rusty nails on the table, peeling paint on the concrete patio, peeling paint on the outdoor benches and table, cracked and broken wagon, cracked drum, cracked pan, three-wheeled trike with tire cracked down the middle.
Evidence: In the kitchen a coffee machine was plugged in and not in use.
Evidence: Child #1, six months old, was observed in a rocker with two blankets covering them.
