Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 11:55am and 1pm. There were 9 children (20 points) directly supervised by the provider and one assistant caregiver. The home, 2 caregiver records, 3 children?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing freely and having lunch. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain a current report of tuberculosis screening every two years.

Evidence:
1. The most recent tb screening in the record for Caregiver #1 was dated 4/20/2021.
Plan of Correction: I will get an updated tb screen.
Standard 8VAC20-800-210-A
Based on record review, 2 of 2 caregivers did not obtain a minimum of sixteen hours of training annually.

Evidence:
1. The record for Caregiver #1 contained documentation of 5 of 16 required annual training hours for the established annual review period.
2. There were no training hours documented during the annual review period for Caregiver #2.
Plan of Correction: I will look for more training to complete.
Standard 8VAC20-800-60-B
Based on record review, 1 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the name and policy number of the child's medical insurance.
Plan of Correction: I will ask the parents for the information.

Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of all required information.

Evidence:
1. The notice of liability insurance form for parents in the record for Child #2 did not contain the parent's written acknowledgement of receipt and date received.
Plan of Correction: I will have the parents sign the form.

Based on record review, the provider did not verify the identity and age of one child within seven business days of the child's first day of attendance at the family
day home as required.

Evidence
1. Child #2 began attending the family day home on 4/10/2023. The record for Child #2 did not contain documentation of viewing proof of identification and age of the child.
