Inspector Notes
An unannounced monitoring inspection was conducted today between 10:39am-12:00pm. There were 3 children directly supervised by the provider. The home, 2 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. The provider was actively engaged with the children during free play. Diapering procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations
5Evidence:
1.Child #1 began attending the family day home on 9/8/2020. The record for Child #1 did not contain documentation of a physical exam.
Evidence:
1. The documentation of a well child visit and immunizations received by the child in the record for Child #4 was not signed.
Evidence:
The provider was observed changing one child's diaper. Afterwards, the provider cleaned the surface and washed her hands. The provider did not wash the child's hands.
Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of all required information.
Evidence:
1. The notice of liability insurance form for parents in the record for Child #2 did not contain the parent's written acknowledgement of receipt and date received.
2. The notice of liability insurance form for parents in the record for Child #3 was signed by the parent, however the form did not indicate whether or not the provider has obtained liability insurance.
3.The notice of liability insurance form for parents in the record for Child #4 did not indicate whether or not the provider has obtained liability insurance, and did not contain the parent's written acknowledgement of receipt and date received.
