Inspector Notes

An unannounced renewal inspection was conducted today from 11:40am-12:30pm. There were 3 children directly supervised by the provider. The home, 1 caregiver record, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the license was not posted on the licensed premises.

Evidence:
1. The current Family Day Home license was not posted in the home.
2. The license posted in the Family Day Home was a previous license issued, which expired on April 12, 2023.
Plan of Correction: Corrected during inspection.
Standard 22.1-289.036-A
Based on record review, the family day home did not obtain the results of a fingerprint-based criminal history record check for 1 household member every 5 years.

Evidence:
1. The most recent fingerprint check results in the record for Household Member #1 was dated 3/5/2019.
Plan of Correction: An appointment was scheduled for 4/15/2024 to complete the background check.
Standard 8VAC20-770-40-D-4
Based on record review and provider interview, the family day home did not obtain a sworn statement or affirmation, a criminal history record check, and the results of a search of the central registry for 1 of 2 household members reviewed.

Evidence:
1. The record for Household Member #2 did not contain documentation of a sworn statement of affirmation, criminal history record check, and central registry check results.
2. The provider confirmed the backgrounds had not been completed.
Plan of Correction: An appointment was scheduled 4/15/2024 to complete the fingerprint check. The provider will provide documentation of completion of all required backgrounds.
