Inspector Notes

A monitoring inspection was conducted on March 8, 2023 from 10:25 am until 11:15 am. There were thirty-four children present aged four months to four years old, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with appliable standards or law and violations were documented on the violation notice issued to the facility.

Based on the review of five staff records, the program failed to ensure that each staff has a completed criminal history check prior to firs day of employment.

Evidence:
1. The first day of employment for Staff #2 was 3/5/2023. There were no criminal history check results in the file.

Based on the review of five staff records, the program failed to ensure that a search of the child abuse and neglect registry was completed for any state in which an individual has resided in the preceding five years for one staff.

Evidence:
1. The first day of employment for Staff #1 was 1/3/2023. There were no results from the child abuse and neglect registry for a previous state of residency.

Based on review of five staff records, the program failed to ensure that each staff member submits documentation of a negative tuberculosis screening at the time of employment for two staff.
