At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section. Staff #1 updated screening was due 10 months ago and the updated screening has not yet been completed.

Program leaders shall have six months of supervised programmatic experience. Staff #2 was working as a lead teacher in a classroom with only three months of supervised programmatic experience.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The last documented fire inspection is dated 11/01/2023 and has not been scheduled to take place.

Annual approval from the health department shall be provided. The last documented health inspection is dated 04/23/2024 and has not been scheduled to take place.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that the resilient surfacing shall comply with minimum safety standards when tested in the American Society for Testing and Materials. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. The fall zones resilient surfacing did not comply with the minimum safety standards.

The center shall implement a monthly practice evacuation drill. There was no indication of a monthly practice evacuation drill for the month of November 2025.
