Staff A's record did not contain documentation of having completed the VDOE sponsored orientation course within 90 days of employment. The training completion was 39 days overdue.

For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child. The record shall contain the amount of time spent on their stomachs.
