The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for Child #2 did not contain any immunization documentation.
Plan of Correction: Going forward center management will ensure that documentation of immunizations are present in children's digital files prior to the child's attendance.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. The record for Child #2 did not contain any documentation of a physical examination. However, the documented first date of attendance is 9/1/2025.
Plan of Correction: Going forward center management will ensure that documentation of physical examination is present in the child's digital record within the required time frame.
Standard 8VAC20-780-200-A
The center shall have a qualified program director or a qualified back-up program director who meets one of the director qualifications who shall regularly be on site at least 50% of the center's hours of operation. There was not a program director qualified staff present during the inspection. Staff present reported that the program qualified staff is present once per week for a couple hours.
Plan of Correction: Going forward center management will ensure that a program director qualified staff is present 50% of the centers operating hours.

Each center shall maintain a record for each child enrolled and contain all required information. The record for Child #1 did not contain the center?s enrollment form which is used to document the required enrollment information.
