Inspector Notes

An in-person monitoring inspection was conducted today. There are 5 children (15 points) with 2 caregivers present. Indoor area, outdoor areas, 2 child, 2 caregiver and 1 household records were reviewed. See Violation Notice for missing items. Emergency policies, supplies and drills were all inspected and in compliance. Violations discussed: expired TB and CPS search results, child record component. and missing CO2 monitor.
The inspection will be typed off site and emailed to the provider.

Based on a review of records, caregiver and household records do not contain documentation of repeated central registry searches (CPS) in the allowable 5 year time frame.
Evidence: 3 of 3 records reviewed contained documentation of CPS searches dated 10/11/2016 (expired)(caregivers A and B, household member c).
Plan of Correction: I will send out the CPS searches as soon as possible.
Standard 22.1-289.058
Based on caregiver statement, there is no CO2 monitor available, in a home built prior to 2015.
Evidence: Provider stated "the monitor was broken and disposed of".
Plan of Correction: I will order a monitor.

Based on a review of records, 1 child record did not contain documentation of a completed physical within 30 days of enrollment.
Evidence: 1 of 2 child records reviewed did not contain a completed physical. First date of attendance is documented as 8/30/2021. (Child 1)
Plan of Correction: I will ask the parent to provide the physical.
Standard 8VAC20-800-170-A
Base on record review, caregiver records did not contain documentation of subsequent tuberculous screening within the allowable 2 year timeframe.
Evidence: 3 caregiver and household records reviewed had tuberculosis results dated 2/17/2020 (expired)(caregivers A and B, household C).
Plan of Correction: We will all go to the doctor.
