Inspector Notes

An unannounced code compliance inspection was initiated on 8/18/2022 and concluded on 8/18/2022 from 10:05 AM to 11:25 AM. There were 15 children present, ranging in ages from three to five, with four staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of three children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure each staff member has been certified by a physician, physician assistant, or nurse practitioner to be free from any disability which would prevent him from caring for children under his/her supervision.

Evidence:

The record for staff 1 documents the start date as 6/4/21. The staff health report is dated 6/7/21.
Plan of Correction: In the future the director will ensure staff provide a staff health report prior to start.

Based on record review and interview, the center failed to establish and implement a procedure to ensure that all staff are able to recognize the signs of child abuse and neglect.

Evidence:

1. The record for staff 1 documents the start date as 6/4/21. The documentation of training in recognizing the signs of child abuse and neglect is dated 6/10/21.
2. The record for staff 3 documents the start date as 9/13/21. The documentation of training in recognizing the signs of child abuse and neglect is dated 9/27/21.
Plan of Correction: In the future the director will ensure staff obtain training in recognizing the sign of child abuse and neglect prior to start.

Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

1. The record for staff 1 documents the hire date as 5/24/21. The fingerprint-based criminal history check determination letter is dated 6/2/21.
2. The record for staff 3 documents the hire date as 9/9/21. The fingerprint-based criminal history check determination letter is dated 9/14/21.
Plan of Correction: In the future the director will ensure staff provide the results of fingerprint background checks prior to hire.
