Inspector Notes

An unannounced code compliance inspection was initiated on 05/23/2023 and concluded on 05/23/2023 from 10:13 AM to 11:30 AM. There were 21 children present, ranging in ages from 2 ? to 5, with 4 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of five children?s records and five staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision prior to start date.

Evidence:

1. The record for staff 3 was reviewed. The hire date is documented as 1/2/23. The staff health report on record is dated 3/31/23.
2. The director verified the date of hire and staff health report.
Plan of Correction: Staff 3's last day at the center was 6/2/23.

Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

Evidence:

1. In the Purple Dino Room (ages 2 1/2 to 3 years) the paint is peeling on the baseboard trim.
2. In the Green Turtle Room (ages 3 to 4 years) Off insect repellant was in a basket on a low shelf accessible to children.
Plan of Correction: The baseboard will be sanded and painted.
The insect repellent will be moved to a higher location.
