Inspector Notes

An unannounced monitoring inspection was conducted on 09/06/2022. There were 34 children, ages 3 months-5 years, and 9 staff members present during the inspection. The inspector observed the following: free choice activities, diapering lunch and preparing for nap. A total of 5 children?s records and 4 staff records, were reviewed. The inspector discussed the following with staff: sick policies and COVID.

Based on observation of the preschool classroom, the center failed to ensure that all chemicals were locked.

Evidence:
1.The chemicals in the preschool classroom were unlocked in the cabinet.
Plan of Correction: During the inspection the teacher locked the cabinet and will ensure it is locked at all times.

Based on review of five children?s records, the center failed to ensure that all were complete.

Evidence
1.Child # 4 did not have two emergency contacts that were not the child?s parents. One of the emergency contacts was the father.
Plan of Correction: The director will have the parent to add a person that is not a parent to the emergency contact list.
