Inspector Notes

An unannounced on-site monitoring inspection was conducted on 3/15/2023. There were 55 children, ages 8 weeks-5 years, and 9 staff members present during the inspection. The inspector observed the following: free choice activities, circle time, lunch and preparing for nap. A total of 4 children?s records and 9 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 9:50 AM and departed at 12:20.

REPEAT VIOLATION

Based on observation, the center failed to ensure that all chemicals were locked in a way that prevents entry.

Evidence:
1.In the infant 2-year-old room and 3-year-old room the locking method did prevent entry into the cabinet where all of the chemicals are stored. The LI could pull the door open when the lock was in place.
Plan of Correction: The owner will obtain new lock for each cabinet that prevents entry.
