Inspector Notes

An unannounced on-site monitoring inspection was conducted on 02/13/2024. There were 50 children, ages 9 months-5 years, and 9 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and painting. A total of 5 children?s records and 3 staff records, were reviewed. The inspector discussed the following with staff: ratio waiver. The inspector arrived at 11:50AM and departed at 1 PM.

Standard 8VAC20-780-140-A
Based on record review and interviews, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s
attendance or within 30 days after the first day of attendance.

Evidence:

The record for Child # 3 contained documentation that the first date of attendance for Child # 3 was 06/13/20222; there was no documentation of a physical exam in the child?s record.
Plan of Correction: The provider will obtain the missing physical ASAP.

2ND REAPEAT VIOLATION

Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.

Evidence:

1.An unlocked Glad air freshener was sitting in a basket next to the bathroom in the preschool classroom.
Plan of Correction: The teacher in the room removed and locked the chemical.
