Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/05/2024 and completed on 08/08/2024. The on-site inspection began at 11AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 86 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review of the medication administration log, the center failed to ensure that all components were obtained when logging medications.

Evidence:

The medication log did not have the name of the staff administering the medication and if there were any adverse reactions or medication errors. Medication is given daily, and the log is dated back to 11/20/2023.
Plan of Correction: Provider will in the future complete all components of the model medication log.
