Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A monitoring inspection was initiated on August 4, 2021 and concluded on August 5, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 52 children present, ranging in ages from five months to five years of age, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of staff files, the facility failed to ensure that at least every two years an updated tuberculosis screening or test was completed.

Evidence:
1. Staff #1's most recent tuberculosis test/screening was dated 01/04/20219.
2. Staff #3's most recent tuberculosis test/screening was dated 03/11/2019.
3. Administration verified the dates of the tuberculosis test/screening were completed, and verified that updates had not been obtained.
Plan of Correction: Administration notified staff and an appointment was made to get the tuberculosis screenings updated.
