Inspector Notes
A monitoring inspection was conducted on September 1, 2022 from 10:00 A.M.-2:00 P.M. There were 58 children present, ranging in ages from 4 months to 4 years of age, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and eleven staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
3Evidence:
1. Staff #3's date of hire was 04/12/2022. The tuberculosis screening/test on file was dated 10/01/2021. The requirement is that documentation of a tuberculosis screening/test being completed must be within 30 days of employment.
2. Staff verified the date of hire for Staff #3, and the date on the tuberculosis screening/test on file.
Evidence:
1. An injury report dated 08/24/2022 was missing the time the parent was notified.
2. An injury report dated 08/10/2022 was missing the time and date the parent was notified.
3. Staff verified that the reports were missing the required information.
Evidence:
1. In Classroom #1 there were three prepared infant bottles in the refrigerator that were not labeled with the child's name. They were on a shelve in the refrigerator with the shelf labeled with the child's name.
2. Staff verified that the individual bottles were not labeled with the child's name.
