During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous
inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/14/2024 and completed on 11/14/2024, as a part of the licensure period.
The on-site inspection began at 11:45am and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 10 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.

This inspection included document review, and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/10/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, and shall have been completed within the last 30 days of the date of employment.
The Tb screenings for four staff were completed more than 30 days prior to the time of employment as follows: Tb for Staff #1 was completed 45 days prior, for Staff #2 was completed 50 days prior, for Staff #3 was completed fifteen months prior, and for Staff #4 was completed twelve months prior.

Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and meet education and programmatic requirements.
The record for Staff #2 did not have documentation of having completed the educational requirements for the lead teacher position.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #3 enrolled for four months, was missing the work phone number of one parent.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
At approximately 12:00 pm on 11/14/2024, there were 8 children present in the toddlers room, but only 3 were marked present on the attendance record.
