Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A monitoring inspection was initiated on 10/22/2021 and concluded on 10/22/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 household member record and 1 staff record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-100-A
The provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child#2's record did not include documentation of a physical examination. The child's first day of attendance was documented as 8/9/2021.
Plan of Correction: Parent is checking with Dr. to see when it will be sent.
Standard 22VAC40-111-180-A
The provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the last screening.

Evidence:
The records for Staff #1 and Household member #2 contain TB screening/test documentation that expired on 3/12/2021.
Plan of Correction: Checking with doctor's office to see when I can have it. Had flu shot, need Covid booster.
