Inspector Notes

A monitoring inspection was initiated with an in-person inspection of the facility on December 22, 2021 beginning at 10:13 AM and ending at 11:00 AM. Records were reviewed off-site via alternative remote protocol. There were 48 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, not all of the areas and equipment inside the center were maintained in a clean, safe and operable condition. Evidence: The children's coats and belongs placed on the steps leading to the upstairs classrooms upon their return inside from recess, creating a tripping hazard. The upstairs bathroom was not clean. The door knob to the half-door of the closet in the 4's classroom was broken and inoperable. The vents in the infant room had a great deal of debris and cobwebs on them.
Plan of Correction: The maintenance man has been contacted about repairs needed by the facility. The staff will ensure that the facility is cleaned. The coats were removed from the stairs by the children during the inspection.
