Inspector Notes
An unannounced monitoring inspection was conducted on 05/29/2024 from 9:50am to 11:35am with the Director. There were 56 children ages 4 months to age 10 years old supervised by 8 staff. The physical plant, outdoor playground area, programming, attendance, menus, 3 staff records, 3 children?s records, 1 medication with corresponding authorization records, 1 allergy care plans, emergency drills, 7 injury reports, emergency supplies, and policies were reviewed. Children were observed participating in free play, board games, watching television and outside play. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations
5Evidence:
1)During inspection in the kitchen directly off the front hallway and where children have access, 1 container of bleach containing a label that stated ?Keep out of Reach of Children? and the word ?Warning? were stored in the open area under the sink in an unlocked area.
2)In the restroom between the Threes and Fours classroom where the door was open, 1 container of antibacterial wipes containing a label that stated ?Keep out of Reach of Children? and the word ?Warning? on the label was stored under the sink in an unlocked cabinet. In the same cabinet there was also an air freshener containing a label that stated ?Keep out of Reach of Children? and the word ?Caution? on the label.
3)In the closet directly off the infant classroom where the door was unlocked, 3 containers of disinfectant wipes and a multi-purpose cleaner containing labels that stated ?Keep out of Reach of Children? and the word ?Caution? on the labels were stored on an open shelf. In the same closet was 1 container of bleach containing a label that stated ?Keep out of Reach of Children? and the word ?Warning? was stored on the open shelf.
The substance was removed and locked in a closet immediately, staff has been instructed it must remain in the locked closet at all times after use.
The infant room has been instructed to keep the back closet door locked after each use. (infant children can not reach the door handle to open the closet door)
1)The Large playground has a merry-go-round with bikes that is 26 inches in height with shredded rubber measuring less than 1 inch in depth measured on the back side of the equipment. The required depth of shredded rubber in this area should be 6 inches.
2)The Large playground has a platform on the back side of the large play structure that is 42 inches in height with shredded rubber measuring at 3 inches on the left side under the platform instead of the required 6 inches of shredded rubber.
3)The Toddler playground has a slide that is 23 inches in height with shredded rubber measuring less than 1 inch at the bottom instead of the required 6 inches of shredded rubber.
4)The Toddler playground has a slide that is 23 inches in height that is placed 42 inches from the side of the building instead of the required 6 foot fall zone.
Evidence:
During inspection while the Threes and Fours classrooms were on the playground, Child #1 left the playground and went inside the building without a staff member. Staff did not escort the child into the building to maintain sight and sound supervision and staff did not notice the child was missing until the child returned to the group on their own.
Evidence: In the Infant classroom, there were 14 containers of food from home that were stored on a shelf that did not have a child?s name on them clearly identifying the owner.
Evidence:
1)During inspection on the playground where the Threes and Fours classrooms were playing, there were 19 children present, but the written record reflected there were 22 children present.
2)During inspection in the Toddler classroom, the written record reflected no documentation of departure for the month of May 2024.
