During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on and completed on 12/4/24. The inspection began at 10:05am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/11/24. A POC submitted after this date will not appear on the public website.
Violations
5Evidence:
During inspection in the infant classroom, Child #1 (10 months old) was sleeping in a crib where the sheets were loosely wrinkled around and under the child. Child #2 (4 months old) was sleeping in a crib with a sherpa blanket covering their face. Safe sleep guidelines require no loose bedding where babies are sleeping due to the risk of suffocation.

Evidence: During the inspection, the center did not maintain the required supervision ratio in the infant room. A staff member left the infant classroom to answer the door and use the restroom leaving 1 staff with 5 infants.

Evidence: During the inspection, the center did not maintain the required supervision ratio in the Twos classroom. There was 1 staff member and 8 children and the youngest child was age 23 months old.

Evidence: The Toddler room and Twos classrooms did not have a sink with running warm water in the diapering area.

Evidence:
1)During inspection in the infant classroom, there were 5 children present, but the written record reflected there were 7 children present.
2)During inspection in the threes classroom, there were 5 children present, but the written record reflected there were 3 children present.
