Inspector Notes

An unannounced monitoring inspection was conducted on 03/20/24 from 9:10 a.m. to 11:09 a.m. with the provider. There were 50 children in care, ranging in age from four months to five-years-old, supervised by 11 staff. The children were observed in circle time and art activities. Five child records and five staff records were reviewed. Two medications and authorization forms were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 07/31/24 and the most recent Health Inspection on file was dated 11/01/23. Areas of non-compliance are identified in the Violation Notice. Repeat Violation

Based on review five staff records; the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.

Evidence: The record for Staff #5 with a start date of 01/04/23, the results of the Central registry search background check was dated 02/23/23.
Plan of Correction: 1/4/22 was the start date the central registry was sent by mail and results were given to us 12 days after the 30 day time frame. Documentation will be added to the staff file at the 30 day mark, in form of an email request to CPS. The email/response from CPS will be added to the staff file at the time of documentation, and every time an inquiry is made if the results are still not returned at the 30 day mark. Using the online portal is more effective than the mailing process that was in 2022, we will continue to use the online portal.
