Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 7/30/24. The on-site inspection began at 10:15 a.m. and ended at 12:30 p.m. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-350-B-2
Based on observation; the facility did not ensure that the ratio for children 16 months to 24 months as 1 staff to 5 children.
Evidence: In the toddler class, (16 months to 24 months), Licensing Inspector (LI) observed 7 children with 1 staff member.
Plan of Correction: The second staff member had taken one child outside with them to set-up for water play just outside the classroom door. Staff have been reminded to maintain ratios at all times even if just a few minutes when setting up or cleaning up from waterplay. If they are unable to take children with them, they will call for staff support to come into the room during these set-up/clean up times.

Based on observation; the center did not ensure that infants placed in their crib were place on their back (supine).
Evidence: In the Infant classroom, (ages 6 weeks- 12 months), LI observed a staff member laying an infant on their stomach in a crib to fall asleep.
Plan of Correction: The infant that was placed in the crib is an infant that is able to roll-over from stomach to back and back to stomach and this information is noted on the crib as well. The LI spoke to the staff and they immediately turned the baby onto their back at the time of inspection. Infant staff was re-trained that all children when placed in the crib will be placed on their back, allowing them to roll over on their own.

Based on observation the center did not ensure that portable wading pools were not used for children that are not toilet trained.
Evidence: The toddler class was observed playing in a splash pad/wading pool at the time of the inspection. None of the toddlers were toilet trained.
Plan of Correction: Toddlers will only use sprinklers moving forward and not a splash pad that allows for "pooling" of any shallow water. Only children that are potty trained and not wearing diapers will be able to use the splashpad that was being used that day moving forward.

Repeat Violation Based on observation it was determined that not all procedures for diapering were followed.
Evidence: In the toddler class, seven clear plastic bags of children?s clothing were observed on top of the diaper changing pad.
Plan of Correction: The children were changed out of their swim clothes into their regular clothes. The teachers had placed the bags with the wet clothes on the changing pad. This was corrected immediately, and the staff removed the clear bags containing the wet items and placed them in the child's personal cubbies. The changing pad was then sanitized prior to using again for diaper changing. Staff have been reminded to place all changes of clothes immediately into the child's personal cubbies whether changing clothes or diapers.
