Inspector Notes

A monitoring inspection was initiated on 12/7/2023 at approximately 9:40am and concluded on 12/7/2023 at approximately 10:50am. There were a total of 5 children with 1 staff members present. Children were observed during circle time and engaged in group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff information, a criminal history records check was not completed for every staff every two years.

Evidence: Staff #1 last had a criminal history record check more than two years ago.
Plan of Correction: Center will ensure the staff member completes a criminal history records check.

Based on review of staff records, it has been more than 2 years since staff has obtained and submitted the results of a follow-up tuberculosis screening.

Evidence: Staff #1's most recent tuberculosis screening was from more than 2 year ago.
Plan of Correction: Center will ensure the staff member obtains and submits a tuberculosis screening result.
