Inspector Notes

A monitoring inspection was initiated on 5/23/2024 at approximately 9:35am and concluded on 5/23/2024 at approximately 11:30am. There were a total of 21 children with 3 staff members present. Children were observed cleaning up, playing with dough and magnet tiles, participating in activity stations, and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation of the playground, center did not ensure areas and equipment of
the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence: Metal clips used to connect the fence and the pole, were on the fence
surrounding the playground and had sharp edges.
Plan of Correction: Center will have the fence repaired.
Standard 8VAC20-780-60-A
Based on review of children's records, the center did not obtain documentation of viewing
proof of the child's identity and age.

Evidence: There was no documentation of viewing proof of the child's identity and age for
Child #2 and Child #3.
Plan of Correction: Center will obtain documentation of the child's identification and age.
